The Georgia Pediatric Program (GAPP) Department of Community Health (DCH) serves eligible children under 21 years of age who are medically fragile and in need of medically necessary skilled nursing care and/or medically necessary personal support. Eligible members should be getting physician ordered services to be considered eligible for the program. GAPP Members Served: Request must be submitted through Medicaid approved GAPP Nursing Agency. We maintain the dignity and respect of clients while supporting their healthy bodily functions.
The time following hospital visits is a critical moment during which we provide up to 14 hours 7 days a week monitoring to insure clients safety and welfare.

Proper nutrition is a cornerstone of maintaining health. We support the proper placement of nutritious foods at the top of the list of priorities for our clients.
Healthy cuisine properly prepared can be both nutritious and delicious. Our professional staff brings food to life with made to order seasonings and cooked to order.
Cleanliness in the bed in which one sleeps can be the difference between health and disease. We take steps to make sure the laundry is both clean and comfortable For clients.
Safe and secure transport of clients is a must. We provide safety, security and style so clients can enjoy the ride and the destination.
Exercise is essential to health recapture, maintenance, and improvement. our supportive staff can assistance the client with proper movement to support improved breathing and movement of muscles, ligaments, bones and internal organs.
